By signing this form, you agree to the following terms:

Automatic Renewal: Your clinic package will automatically renew each month unless a cancellation form is submitted.

Cancellation Policy: To cancel your package, you must complete and submit a cancellation form no later than the 20th of the current month to avoid charges for the following month.

Billing Authorization: You authorize Sunset to process payment for each renewal cycle unless a cancellation form is submitted by the deadline.

No Refunds: Payments are non-refundable, and missed sessions cannot be credited or carried over to future months.

Attendance Beyond Package:
If a player has used all sessions in their package but attends an additional clinic, they will not be turned away. The daily clinic rate will automatically apply.
