Individual Release and Hold Harmless Agreement:
I understand that participating in programs with Campton Township involves a certain degree of risk that could result in injury, loss, or damage to person or property. After carefully considering the risk involved, I hereby release, hold harmless and waive all claims associated with this activity which I, or my minor children, may have against Campton Township, its employees, elected officials and volunteers. There are also certain risks of physical injury in connection with the
administration of medication or use of auto-injectors and inhalers. Such risks can include failure to properly administer medication, failure to observe or recognize adverse reactions or medical emergencies, and failure to recognize the need to summon emergency medical services. In this regard, it is impossible for the Campton Township to guarantee absolute safety.
I further understand that providing health or medical information allows Campton Township staff and volunteers to share that information with emergency personnel in the case of a medical emergency. I give Campton Township staff permission to transport my child/ward for emergency treatment if it is impossible to reach me.
Media Release:
I hereby authorize Campton Township to use any photographs and/or videos taken of myself and my minor children for newsletters, publicity, appeals and other publication. This applies to both candid and posed photographs and/or videos. I further authorize the use of information gathered from me during any interview by employees of Campton Township. This agreement covers all future potential use of said photographs, videos or information. I hereby give permission for my name and the names of my minor children to be used in conjunction with the photographs and/or any written materials.
Preparing for Camp:
Campers are asked to bring a sack lunch, snacks, reusable water bottle, bug spray, and sunscreen each day to camp (bugspray and sunscreen to be applied before camp). Campers must wear sturdy close-toed shoes each day as we will be out and about in the park.(campers will not be allowed to attend if they have open-toed shoes on.) Campers should be prepared for the weather and may wish to bring a hat and/or rain jacket if needed. This is an outdoor camp, we will spend some time indoors but most of our time will be outdoors. Please send your child with a backpack/bag with essential items inside.
CANCELLATION POLICY: The Full Refund date is the Monday, two weeks prior to each camps week. After that date there will be No Refunds, no exceptions. We have a lot of preperation for these camps and shirts are ordered ahead of time, therefore we cannot accomodate late cancellations.
