This form is to request an appointment ONLY - once the form has been accepted, Fawn Tooth will contact you by email to discuss the booking process - if both parties agree to go forward with officially booking the tattoo appointment, a non-refundable deposit of $50.00-$100.00 will be sent to: fawntoothtattoo@gmail.com which will be taken off the final price of the tattoo and the appointment will be reserved - if there are any last minute cancellations or rescheduling with notice of more than 48 hours, the deposit will continue forward to a new appointment which will be agreed upon by Fawn Tooth and the client, with the amount of the deposit being taken off the final price of the tattoo - any client who does not show up to their agreed upon appointment, or give insufficient notice, will have to pay another deposit to book a different appointment.
