IMPORTANT INFORMATION

By turning in this form you are confirming that your team has a committment by the minimum number of players for the entire Championship. You are also confirming that, if selected, your team will be able to register the minimum number of players and place the team deposit within 24 hours of receiving notification from the Tournament Director. 

TEAMS THAT TURN IN THIS FORM AND DECLINE THE INVITE WILL NOT BE ELIGIBLE FOR A WILDCARD IN FUTURE TOURNAMENTS.

If you team is unable to accept a wildcard, let your Local League Coordinator know that you need to removed your name from consideration.

55/70 & Over:

April 29 - Deadline for Wildcard Jotform to be turned in

April 30 - Teams will be notified if they did or did not receive a wildcard

May 1 - Wildcard teams that were selected must have the minimum number of players registered and the deposit on file.      

40 & Over:

May 13 - Deadline for Wildcard Jotform to be turned in

May 14 - Teams will be notified if they did or did not receive a wildcard

May 15 - Wildcard teams that were selected must have the minimum number of

18 & Over:

May 27 - Deadline for Wildcard Jotform to be turned in

May 28 - Teams will be notified if they did or did not receive a wildcard

May 29 - Wildcard teams that were selected must have the minimum number of players registered and the deposit on file.
