Approval & Agreement
By submitting this form, you agree to the following:

✅ Prices for each product will be discussed once the base cost of the design is determined.
✅ Products must be priced at least $5 above the base cost.
✅ The League of Chicago Theatres will receive 30% of the profit from each item sold.
✅ The League will provide mockups for final approval before launching sales.
✅ The theatre will receive payment only after funds are received by the League of Chicago Theatres, typically by mid-month for the previous month's sales.
