I hereby give my approval to his/her participation in any and all program activities. I assume all risks and hazards incidental to such participation including transportation to and from the activities; and do hereby waive, release, absolve, indemnify and agree to hold harmless the JCC of Northern NJ, JCC Rockland, its affiliates, the organizers, sponsors, supervisors, participants and persons transporting him/her to or from activities, for any claim arising out of injury to him/her, except to the extent and in the amount covered by accident or liability insurance. In an effort to share meaningful and impactful moment in our community, the JCC may occasionally record photographs, audio recordings and/or videos of people at our programs and published in promotional materials, including but not limited to web, social media and newspaper articles (“Promotional Materials”). I agree to allow the JCC of Northern NJ and JCC Rockland to use Promotional Materials of myself and/or my family. We look forward to showcasing your smiling faces.
