Before completing the Parental Consent Form below, please ensure you have read through our DHA Family Handbook if you have not already done so. A copy of the handbook will also be emailed to you upon confirmation of your enrollment. After reading through the handbook, be sure to come back to this form to complete and submit your enrollment.
This consent form is hereby voluntarily signed as parent or legal guardian for the child(ren) listed as part of the application for enrollment to Discovery Homeschool Academy (DHA). The undersigned parent or guardian agrees and consents to the terms herein. Please read through each of the following statements and check each box to agree.
Please complete your enrollment fee payment below. Enrollments are taken in the order that enrollment fee payments are received.
Please note: Enrollment fees are non-refundable. The only exception to this policy is: If your family is placed on a waiting list and is still on the waiting list by the time school begins, then your family may email to request a refund of your enrollment fee at that time if you do not wish to remain on the wait list throughout the school year.
