    Summer Day Camps 2025 Registration Form

    Please fill in all required* information for registration. One camper per form.
  • Thank you for your interest in registering your child or children for summer day camp with Carpe Diem Arts. We are excited about this summer and our plans for each camp.

    Please note that a separate form must be completed for each child.

    2025 Day Camp Fees - $395 per week

    Camps run from 9:30 am to 3:30 pm Monday through Friday.
    Each camp ends with a celebration with families and caregivers
    at the end of the day on Friday, usually around 2:30 pm.
    Please add this date to your calendar, 
    as your child will be eager to show off all they have done!

    Add-on option:  $15/hr for before and after care of no more than one hour each.
    Available for elementary grade camps only.

    Campers must bring their own lunch and water bottle - snacks are provided.

    • My child will follow all directions of camp personnel with respect to our health precautions/ protocols, social distancing, and general safety.
    • The camp is not responsible for any damage to or loss of cell/mobile phones or other items brought to the camp.
    • For camps other than grades 1-2, if your child does not have access to a ukulele, one will be provided by the camp for use throughout the week. The child is responsible for its care and safe-keeping while attending the camp.
    • The camp is not responsible for administering any medication to a child that he/she/they take on a regular basis. Medicines of any kind, including prescription drugs, will be the responsibility of the named parent or guardian.
    • I hereby release Carpe Diem Arts, Inc. and any and all staff and volunteers from any and all liability with relationship to participation in Carpe Diem Arts' camp program. Further, I hereby release from liability the property owners upon whose land and facilities the camp will be located. If my child is injured and requires medical attention, and I cannot be reached for instructions, I do hereby give authority to the aforementioned parties to obtain the necessary emergency medical treatment for my child by EMT and/or at the nearest hospital with the understanding that the family will be notified as soon as possible and will be responsible for any charges incurred.
    • I understand that the Camp does not provide accident/medical insurance for campers. I hereby certify that my child's immunizations are up to date, including tetanus.
    • No information entered on this form will be shared with any other organization.
    • The Camp has my permission to videotape and/or photograph my child while at the Camp. Images including my child may be used for operational training, marketing and fundraising purposes.

    Check must be received within 10 days of registration or camper's reservation will be released.

     

    TO PAY BY CASH - Complete this as if you are paying by check but put CASH in the check number field.  You MUST bring cash on the first Monday morning of camp.
