  • Completion of this form is required to be considered for intake and submitting a request does not guarantee intake. Failure to complete this form in its entirety will result in delays in processing your request and may result in your request not being evaluated.

    If a surrender request is accepted and an appointment made with us to surrender the animal but no communication is received or no one shows, the intake slot will be given to next animal on our intake list.

    We know the decision to surrender your pet is a very difficult and emotional one. Thank you for trusting us with them. Our primary mission is to take the best care possible of every animal in our rescue.
