ASHI Travel Award Application

The ASHI Travel Fund financially assists ASHI members so that they may attend ASHI meetings. If you would like to be considered for a travel award (to be reimbursed following the meeting), please complete this application. Your application will be reviewed and you will be notified if you are approved to receive a travel award.

NOTE: If you have previously been awarded an ASHI Travel Award, you are no longer eligible to apply.
