Permission: I grant Katabolic Magazine and their legal representatives the irrevocable and unrestricted right to use and publish photographs of me, or in which I may be included, for editorial, trade, advertising, educational, and any other purpose and in any manner and medium; to alter the same without restriction; and to copyright the same.
Use and Disclosure: I agree that Kataolic Magazine may or may not use such photographs of me with or without my name and for any lawful purpose, including, but not limited to, publicity, illustration, advertising, and web content.
Release and Waiver: I release and discharge Katabolic Magazine from any and all claims and demands that may arise out of or in connection with the use of the photographs, including without limitation any and all claims for libel or violation of any right of publicity or privacy.
Ownership: I acknowledge and agree that I have all ownership rights in the photographs I am submitting and that I have the Permission from the Photographer/Model to submit these images for publication.
Compensation: I understand that I will receive no compensation for the use of the photographs.
Personal Information: I understand that my personal information may be used in connection with the use of the photographs.
Minor Model Release (if applicable): If the model is a minor, the form may include a section for the parent or legal guardian to provide consent.
