Sprout & Shine Liability Waiver
Let’s Play, Learn, and Grow!

By signing this Liability Waiver, I acknowledge and agree to the following terms and conditions regarding my participation (or the participation of my child/ward) in Sprout & Shine classes, events, and activities:{studentName}

1. Assumption of Risk

I understand that participation in Sprout & Shine activities, including but not limited to sports, art, yoga, STEM, and other enrichment programs, involves physical activity and potential risks, including but not limited to minor injuries (e.g., bruises, scrapes) and, in rare cases, serious injury or illness. I voluntarily assume all risks associated with participation.

2. Release of Liability

The Participant (or Parent/Guardian, if the Participant is a minor) agrees to indemnify, defend, and hold harmless Sprout & Shine, its owners, employees, contractors, volunteers, affiliates, and agents (collectively, the "Released Parties") from and against any and all claims, liabilities, damages, losses, costs, or expenses (including, but not limited to, attorneys' fees) arising out of or in connection with:

1. Any injury, illness, damage, or loss sustained by the Participant or any third party as a result of participation in any Sprout & Shine activity, including but not limited to sports, enrichment classes, and events.

2. Any negligent, reckless, or intentional act or omission by the Participant (or Parent/Guardian).

3. Any violation of the terms of this agreement by the Participant (or Parent/Guardian).

This indemnification obligation extends to all claims, including those based on the alleged negligence of the Released Parties, to the fullest extent permitted by law.

By signing this agreement, the Participant (or Parent/Guardian) acknowledges their understanding of this indemnification clause and accepts full responsibility for any risks associated with participation in Sprout & Shine activities.
