Agreement & Consent

1. Assumption of Risk

I understand that participation in Sprout & Shine activities (including sports, art, yoga, STEM, and enrichment programs) involves physical activity and inherent risks, including injury or illness. I voluntarily assume all risks.

2. Release of Liability

I agree to release, indemnify, and hold harmless Sprout & Shine Kids LLC, its owners, employees, contractors, volunteers, and affiliates from any claims, damages, or injuries arising from participation, including those caused by negligence to the fullest extent permitted by law.

3. Medical Authorization

I authorize Sprout & Shine staff to obtain emergency medical care for my child if necessary. I understand I am responsible for all medical expenses.

4. Supervision & Behavior Policy

I understand I must remain on-site during activities and am responsible for supervising my child.

I understand disruptive or unsafe behavior may result in removal without a refund.

5. Personal Belongings

I understand Sprout & Shine is not responsible for lost, stolen, or damaged items.

6. Photo & Media Release (Optional)

I give permission for photos/videos to be used for marketing and social media

I understand classes are often outdoors and I am responsible for proper clothing.

I understand classes are only rescheduled for severe weather and are non-refundable. Check Facebook for canceled sessions if weather is bad.

8. Snacks & Allergies

I understand snacks may be provided and I am responsible for checking ingredients.

I release Sprout & Shine from liability related to allergies or food reactions.

9. Refund Policy

I understand all payments are non-refundable.

10. Ongoing Agreement

I understand this waiver applies to all future Sprout & Shine activities unless revoked in writing.

11. Acts of God & Unforeseen Events

I understand and agree that Sprout & Shine Kids LLC is not responsible or liable for injuries, damages, losses, or death caused by events beyond its control. These may include, but are not limited to, acts of God, severe weather, natural disasters, accidents, or other unforeseen circumstances.

I acknowledge that participation may still occur in outdoor or uncontrolled environments and I voluntarily accept all associated risks.

12. Family Coverage Agreement

I agree that this Liability Waiver applies to all minor children under my care, custody, or control who participate in Sprout & Shine activities, now or in the future.

I represent and warrant that I have the legal authority to execute this agreement on behalf of each child and bind them to its terms.
