  • Do you agree that in extreme circumstances, your dog will be placed in boarding kennels?*

  • Note that this is a licence requirement for extreme emergencies (e.g. cases of aggression, a fire in the boarder’s home etc). If your Emergency Contact will not collect your dog (and you cannot find anyone else to pick up). There may be fees to pay at the Kennels when you collect your dog. This is a last resort only and not a normal occurrence.

  • Media & Photo Consent

    At Bonnie’s House, we may take photos and videos of dogs in our care for record-keeping, updates, and monitoring. These images may also be shared privately with owners.With your express consent, we may also use some photos/videos for our website, social media, or marketing materials. We never share dogs' names in public posts.

    I hereby confirm that I am the owner of the dog named on this form and that I authorise Bonnie’s House to act as my dog’s guardian whilst providing the services.

    I confirm that I have read, understood and signed the Bonnie’s House Service Agreement in conjunction with this Registration and Consent form.

    I attest that the information I have provided in this document is an accurate account of my dog’s needs and behaviour. I am aware that if my dog behaves in a manner which is dangerous to a person or another dog in Bonnie’s House care, I will be contacted immediately and must address the issues before the commencement of further services. I undertake to keep Bonnie’s House informed of any changes in my dog’s health, behaviour, or medical needs before making each new booking.
