2025 EMDSL OUTDOOR REGISTRATION FORM

You will need to fill out one registration per TEAM.

Tier 1/2 Registration deadline is Sunday March 30 2025.
Tier 3 Registration deadline is Sunday April 13th 2025.

Registration fee: Tier 1 and 2 $300 per team Tier 3 $200 per team. Fees must be paid by club cheque. Drop off or mail to 295 Rectory Street London ON N5Z 0A3

Age Divisions may be combined where there are not enough teams to form a single age division. Tier 3 upper ages are double year cohorts, U14, U16 and U18.

Please ensure that you have read the registration instructions document on the league website prior to completing this form. This year we are asking for the year of birth of the oldest player on the team to ensure the team is being registered to the correct age division. Teams are not allowed to play up in grassroots divisions but can request to play up in U13 and older divisions. For example a team wishing to play up from U13 to U14 would enter 2012 as the oldest player on the team year of birth and then 2011 as the division year of birth the team wants to play in. Teams or players cannot play down.
