    Welcome to CareBridge Academy, where your journey to becoming a Certified Nurse Aide begins! 

    Please note that enrollment in our program is not guaranteed, as students must meet certain qualifications to enroll.

    The following steps encompass all components of initial registration and enrollment. We encourage you to review them carefully and ensure that you meet the necessary requirements before proceeding with your application. 

    Be sure to check the current schedule and know which one of our locations you are interested in:

    • Darby, Philadelphia (Delaware County) branch, located at 1201 Springfield Rd., Darby, PA 19023 (inside of Little Flower Manor) offers our morning program (8:30 AM to 3 PM). 
    • Northeast Philadelphia branch, located at 8580 Verree Rd, Philadelphia, PA 19111 (inside of Redeemer Lafayette) offers our morning program (8:30 AM to 3 PM).
    • Central Philadelphia branch, located at 448 N. 10th St., Suite 400, Philadelphia, PA 19123 offers our morning program (8:30 AM to 3 PM) and also our evening program (4:00 PM to 9:15 PM).

    At any point, you can save your submission and return to it later just by hitting the "save" button.

  • Entrance Exam

    Dear Applicant,


    Welcome to the admissions process for our institution. We're excited to embark on this journey with you towards academic excellence. Before proceeding further, there are some important details regarding the entrance exam that you need to be aware of. 

    The entrance exam is a dual evaluation in our admissions process. It comprises 10 math questions and 20 reading questions designed to evaluate your English language competency. The  first part of the exam is timed for 30 minutes, so it's crucial that you manage your time effectively. The second part is administered by the office coordinator after the first part is done. 


    Here are some key points to remember about the entrance exam:

     

    Content: The exam consists of 10 math questions and 20 reading questions. Materials: You are allowed to use a calculator during the exam, but no other materials or guides may be used.


    Passing Criteria:

    For the math section, you must correctly answer at least 7 out of the 10 questions (70%).
    For the English comprehension section, you must correctly answer at least 14 out of 20 questions (70%).
    Retest Policy: If you do not pass the exam, you must wait at least 14 days before you can retest.

    We urge you to take the exam seriously and only proceed when you are ready. It's essential to give your best effort and showcase your skills during the assessment.

    DO NOT PROCEED UNTIL YOU TAKE AND PASS THE EXAM. IF YOU DO NOT TAKE OR PASS THE EXAM YOUR APPLICATION WILL BE DISQUALIFIED.

     

    IF YOU FAIL THE EXAM AND ATTEMPT TO RETAKE IT YOUR APPLICATION WILL BE DISQUALIFIED.

  • Please verify below the status of your exam that you took before proceeding. Please note that all tests are reviewed by Admissions staff and if you continue to register for the program after failing the exam your registration will be rejected.

    By typing my name below, I attest that the entrance exam was successfully completed and resulted in a passing score. I am aware that if I proceed with the application process but did not pass the entrance exam, or if I retook the entrance exam before the allowable date after exam failure that my registration will be null and void until such requirements are properly met.

    I am delighted to extend my warmest congratulations to each of you for successfully passing the initial entrance exam for our Nurse Aide Training Program. This accomplishment marks the first step in your journey towards joining our esteemed program. 

    Be reminded that passing the entrance exam is just the beginning of the process. The next important step is completing paperwork required for enrollment. It's imperative that you carefully review all the information provided and ensure that you sign and date where requested. Your enrollment is not confirmed until all of the paperwork is completed in full. Please read the information carefully.

  • Identification Verification- 2 Forms Required

     

    In the following section, and as part of program requirements, you must upload two forms of identification. Identification must meet the requirements below:

    • Proof of current PA residency such as a Driver’s License or state ID
    • Both IDs MUST contain your signature; one photo ID is required

    Acceptable forms of identification include the following. We cannot accept any document(s) that are expired:

    1. PA Driver’s License
    2. PA State ID
    3. Passport
    4. Military ID
    5. Signature bearing Library/CPR Card
    6. Resident Alien Card/Permanent Residency Card
    7. Voters Registration
    8. Vehicle Registration
    9. Certificate of Naturalization
    10. Birth Certificate
    11. Social Security Card

  • COVID Vaccination Proof

     

    If you have a record of your COVID vaccination history, such as a medical record or card, please upload that below. If you do not have that available or need an exemption form, that can be obtained or uploaded at a later date. 

     

    PLEASE NOTE: Proof of previous COVID vaccination (at least one step), is required at our Darby and Northeast Philadelphia locations.

    Dear applicants,


    Now that you have completed the enrollment application we will now proceed to reserve your space in the program. A $250 enrollment fee is required. YOUR APPLICATION WILL NOT BE CONSIDERED OR REVIEWED UNTIL THIS PAYMENT IS MADE. This fee includes both the enrollment fee and supplies fee.

    For third party entities paying for your program: If a third-party entity will be paying for your spot, (such as PA Careerlink, a nursing facility, scholarship fund, etc.), please indicate that below and you will not need to pay the fee to reserve your spot once we have confirmation of your funding source.

    Please complete your payment using ONE of the links below.  Failure to complete any payment will result in an incomplete application status and your application status will not be reviewed or reserved until this payment is made.
