• InterRAI CHA Course Registration Request

  • Thank you for your interest in attending the North East interRAI Community Health Assessment (CHA) Full Course.

    Completion of the Request for Registration Form does not guarantee registration.

    Registration confirmations will be sent out by email.

    Course Requirements:

    1. The participant will have Electronic Assessment Software System: A unique user account in a licensed electronic software system for the interRAI CHA assessment that uploads assessments to the Integrated Assessment Record (IAR)

    2. The participant will attend all 3 days of the course

    3. The participant will have a reliable internet connection, a webcam, and a microphone.

    4. The participant will have a copy of the required Manuals in time for the course date

  • Does the organization already have the ability to upload assessments to the IAR?*
  • Does the participant already have their own account to upload assessments to the IAR?*
  • Will the participant have a reliable internet connection, their own webcam, and microphone?*
  • The participant must attend all three days of training in their entirety, and participate in all discussions. Will the participant be able to do so?*
  • Will the participant complete and upload an InterRAI CHA assessment in the first 3 weeks following the training?*
  • Do you agree to send a screen shot of the the IAR showing the participant's first completed assessment?*
  • Does the participant have access to or is the organization able to procure the InterRAI Community Health Assessment Form and User's Manual Version 9.1.4 and the InterRAI Clinical Assessment Protocols (CAPs) Manual prior to training?*
