Waiver and Release of Liability for Upward PH Coaching
Please read and agree to the following terms and conditions as a participant in the 1-on-1 coaching sessions provided by Upward PH
Voluntary Participation: I understand that participation in jump rope coaching sessions is voluntary and involves physical activity that may carry inherent risks of injury.
Assumption of Risk: I acknowledge that I am physically fit to participate in these activities and assume all risks associated with my participation, including but not limited to sprains, strains, or other injuries.
Release of Liability: I release and hold harmless Upward PH, its coaches, agents, and representatives from any and all liability, claims, or demands arising from my participation in the coaching sessions.
Medical Emergencies: In the event of an injury or medical emergency, I authorize the coaches of Upward PH to seek medical assistance on my behalf and agree to bear any associated costs.
Compliance with Instructions: I agree to follow all instructions provided by my coach and use any equipment properly to minimize risks.
Confidentiality and Media Release: I consent to the use of photos or videos taken during sessions for promotional purposes unless I explicitly notify Upward PH otherwise.
