🔹 All schedule adjustments and withdrawals must be submitted via this form by February 7 to be considered. Requests via email, phone, or text will not be accepted.
🔹 Students without a submitted form by February 7 will be confirmed for Spring Enrollment, and tuition will be charged to the stored payment method.
🔹 Please use the same email address that receives Pike13 notifications for the primary adult contact on your account.
🔹 If requesting changes for multiple students, please submit a separate form for each student
🔹 It will take up to two weeks to process all the change requests before you will see a confirmation.
🔹 Change requests are not confirmed until you have received your enrollment notification through PIke13. If we are unable to accommodate your request, you will receive a call or email.
