Here are some important things you need to do before completing this form:
Review and confirm you meet the criteria:
- Applicants must be a dependent of a first responder who resides and either worked full-time and is now retired in Monmouth County or is currently employed as a full-time first responder and lives in Monmouth County.
- A legal guardian must be an active member of the 200 Club of Monmouth County.
Review submission requirements:
(1) A copy of your transcript – an unofficial copy is acceptable.
(2) A copy of the legal guardian’s current 200 Club Membership (copy of the card or proof of payment).
(3) Proof you are enrolled to attend a credited learning institution Fall 2025 - Proof can be a copy of your canceled check, credit card receipt, or a letter from the institution confirming receipt of payment.
