Participant Enrollment Form

Victory Sports Camp is a summer camp for students who have completed 1st-6th grade.

Venue: Colleton County Middle School (1379 Tuskegee Airmen Dr, Walterboro, SC)

Date: July 21-25

Time: 9 am-4 pm

Grades: *1st-6th grade (completed)
*must turn 7 before September 1, 2025.

Fee: $25

Participants will choose one sport from the following list:
Baseball/Softball (bring your glove- we have extras if needed)
Soccer (shin guards required- we have extras if needed)
Football
Basketball
Cheer
Volleyball
Please note that campers will remain in the same sport throughout the week and cannot switch sports once camp begins.

Camp Schedule:
9 am-noon: Sports Camp
Noon-1 pm: Lunch (provided)
1 pm-4 pm: Vacation Bible School

Fee:
Cash or check. If paying by check, please make payable to the following: Colleton Baptist Association.

Refund Policy: The parent/guardian should notify Victory Sports if their child cannot attend camp. You will be issued a full refund if notified before or during the week of camp. After this time, we are unable to process any refund. Refunds are not given for missing a day(s).

Release of Liability
I have been informed of the above activity sponsored by Victory Sports Outreach, Inc. (VSO). Therefore, I now consent for my minor child to participate in this activity. I understand that the leaders of this activity will take all reasonable safety precautions and that the possibility of an unforeseen hazard does exist. Therefore, I further agree not Colleton County Middle School, Colleton School District, partner churches/sponsors, or VSO employees and volunteer staff liable for damages, losses, diseases, or injuries by the minor listed on this form. I consent to any medical treatment that may be deemed necessary for my minor child. Further, I understand that all efforts will be made to contact me before treatment. If I cannot be reached in an emergency, I permit the activity leader to make the necessary decisions for treatment. Should there be no activity leader available, I authorize the attending physician to treat my minor child. Finally, I understand that the doctors, dentists, and other providers attending to my child will take all reasonable safety precautions during their care. Therefore, on behalf of myself and my heirs, administrators, and executors, I, at this moment, release and hold the Victory Sports Outreach and its officers, partner churches, employees, volunteers, agents, and contractors harmless from all such claims for injury and damage. I understand that I would not be permitted to participate in any VSO program or use any VSO or partner facility or equipment without acceptance of this agreement.
