I acknowledge that beauty and wellness treatments, the practice of skin care, and the practice of advanced esthetic procedures, including, but not limited to, Lash Extensions, Keratin Lash Lifts, KLOUD PEMF Devices and various other beauty procedures is not an exact science and no specific guaranties can or have been made concerning the outcome. I understand that some clients experience more change and improvement than others. In virtually all cases, multiple treatments are required in order to realize a difference.
I also understand and agree to assume the following risks and hazards which may occur in connection with any particular treatment including but not limited to: unsatisfactory results, soreness, poor healing, discomfort, redness, blistering, nerve damage, scarring, infection, change in skin pigmentation, allergic reaction, muscle damage, and increased hair growth. I understand that even though precautions may be taken in my treatment, not all risks can be known in advance.
Given the above, I understand that response to treatment varies on an individual basis and that specific results are not guaranteed. Therefore, in consideration for any treatment received, I agree to unconditionally defend, hold harmless and release from any and all liability the company and the individual that provided my treatment, the insured, and any additional insureds, as well as any officers, directors, or employees of the above companies for any condition or result, known or unknown, that may arise as a consequence of any treatment that I receive.
Upon entering Love Cheni The Beauty Boutique and Wellness Center, I acknowledge that I am aware of the risks involved from receiving treatment and I voluntarily agree to assume those risks, and I release and hold harmless Jane Nguyen and Love Cheni The Beauty Boutique, Dr. FeelGood and Centropix from any claims related thereto. I understand and agree that any legal action of any kind related to any treatment I receive will be limited to binding arbitration using a single arbitrator agreed to by both parties. I give my consent to receive treatment from Jane Nguyen.
Regarding the KLOUD, None of the statements in the Centropix, Love Cheni or Dr. FeelGood marketing and educational materials or websites have been evaluated by the Food and Drug Administration (FDA). They are not intended to diagnose, treat, cure or prevent any disease.
Furthermore, none of the statements should be construed as dispensing medical advice, making claims regarding the cure of diseases, nor can these products prevent or cure any disease state.
Centropix, Love Cheni or Dr. FeelGood products are in no way a substitute for medical care. You should consult a licensed healthcare professional before starting any health protocol or any health device such as the Centropix Kloud, especially if you are pregnant or have any pre-exist- ing injuries or medical conditions.
You hereby release and hold Dr. FeelGood, Love Cheni, Centropix USA and its parent, subsidiar- ies, affiliates, partners, officers, directors, agents, employees, contractors, service providers, or suppliers (”us”) harmless from any and all claims, demands or causes of action of any nature and kind, known or unknown, which you or somebody on your behalf has or may in the future have against us relating directly or indirectly to your use of Centropix, Love Cheni or Dr. FeelGood products. Should this provision be found unenforceable under applicable law, the remaining provisions will remain in full effect.
By signing and filling out this form, you have acknowledged that you have read, understood and agree to all of the above.
