Dog Trainer Waiver

- The Owner certifies that their pet is in good health and up-to-date on all vaccinations, including rabies, distemper, parvovirus, and Bordetella.

-The owners confirm that their pets flea and tick treatment is current.

-It is understood by the owners that Service Provider is not responsible for injuries involving pre-existing or age-related conditions.

-In case of an emergency, the Service Provider is authorized to seek veterinary care for the pet at the Owner's expense.

- The Service Provider will make reasonable efforts to contact the Owner or the emergency contact provided.

-The Service provider is authorized to approve medical treatment up to $3000.00 in the event the Owner cannot be reached during a medical emergency, the dogs owner will be responsible for reimbursing said cost as well as paying for future care associated with their dogs care.

-The owners confirm that any behavioural or medical issues have been disclosed to the Service Provider, and that their dog does not have a communicable illness.  This includes disclosing if there is a bite history (person or animal), resource guarding issues, reactivity issues, escape/runaway tendencies (over/under fences, through doors), shown aggression with other dogs, if they are spayed or neutered.

-The owners acknowledge that there is an inherent risk of illness or injury when dogs are outdoors, or in a group situation, and nicks, scratches or other ailments aren’t unusual.

-The owners acknowledge that pets are instinctual in nature and not always controllable even with the highest level of care provided.  

-The owner confirms that their pet is licensed with the City of Calgary and that the license is up to date.

-The owner authorizes the Service Provider to enter their home to pick up or drop off their pet for their training.

-The owner agrees that the Service Provider is not responsible for any damage to their home or property caused by their dog.

-The owner consents for their dog to be involved in pack walks, and they consent for offleash walks (in a fenced private yard or an off-leash area designated by the municipality)

- The Owner acknowledges that there are inherent risks associated with dog training.

-The owner acknowledges their dog will be around other dogs in the service providers care.

-The owner assumes responsibility for any harm or damage caused by their dog.

- Dog training is not a guarantee against future aggression or biting incidents

- The owner consents to the use of any training tool

- The owner acknowledges that there is not always a “cure” for behavioural issues and the Service provider cannot guarantee any outcome

- The Owner agrees to hold the Service Provider harmless for any injury, loss, or damage that may occur while the pet is in the Service Provider’s care, except in cases of gross negligence or willful misconduct.

-Invoices will be emailed to owner for dog training and a deposit is required the same day as invoice is received, the remainder of payment is required on the first day of service provided.

-The owner consents to the use of social media. This shall include photos of the owners dog on various social media apps including social media advertising campaigns, training videos as well as daily activity posts.

-  Cancellations must be made in writing two (2) weeks in advance.

-  Late cancellations may be subject to a cancellation fee.

- By signing below, the Owner agrees to all terms and conditions outlined in this waiver.
