The individual or sole proprietor applying must demonstrate that they are based in the Appalachian Ohio region by supplying a physical billing address at the time of application. The county region includes Morgan, Athens, Perry, Hocking, Fairfield, Vinton, Meigs, Washington, and Muskingum.
