Media Release
I consent to the photographing and video recording of myself and reproduction and/or use of these photographs and/or recordings in whole or in parts, and alone or with other products, for advertising, publicity, or other commercial purposes domestically and internationally in perpetuity.
I hereby release Movers and Shakers Salsa and Bachata Dance Academy, Daniel Kalman, and all other associated instructors or assistant instructors and any of their associated or affiliated companies, their directors, officers, agents, employees and customers, and appointed advertising agencies, their directors, officers, agents and employees from all financial claims and claims of every kind on account of such use.
Liability Release
I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. Furthermore, I understand that students will be in close contact with one another and there are risks associated with contracting the Covid-19 virus. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Movers and Shakers Salsa and Bachata Dance Academy, Daniel Kalman, it’s instructors, directors, assistant instructors, officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “The Company”).
I hereby agree to release The Company and hold The Company harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance.
I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
