4-Week Cancellation Notice Form

We're sorry to see you go! Before submitting your cancellation request, please review the information below regarding Study Buddy's 4-week cancellation notice policy.

𝗜𝗠𝗣𝗢𝗥𝗧𝗔𝗡𝗧 𝗜𝗡𝗙𝗢𝗥𝗠𝗔𝗧𝗜𝗢𝗡 𝗥𝗘𝗚𝗔𝗥𝗗𝗜𝗡𝗚 𝗬𝗢𝗨𝗥 𝗖𝗔𝗡𝗖𝗘𝗟𝗟𝗔𝗧𝗜𝗢𝗡

• 𝗙𝗼𝘂𝗿-𝗪𝗲𝗲𝗸 𝗡𝗼𝘁𝗶𝗰𝗲: Your subscription will remain active for four weeks from the date this form is submitted.

• 𝗣𝗮𝘆𝗺𝗲𝗻𝘁𝘀 & 𝗦𝗲𝘀𝘀𝗶𝗼𝗻𝘀: All scheduled sessions and payments will continue throughout the four-week notice period.

• 𝗥𝗲𝗳𝘂𝗻𝗱𝘀 & 𝗖𝗿𝗲𝗱𝗶𝘁𝘀: Refunds, credits, and prorated adjustments will not be provided for unused sessions during the notice period.

• 𝗨𝗻𝘂𝘀𝗲𝗱 𝗦𝗲𝘀𝘀𝗶𝗼𝗻𝘀: Any remaining sessions you choose not to attend will be forfeited.

• 𝗣𝗮𝘆𝗺𝗲𝗻𝘁 𝗗𝗶𝘀𝗽𝘂𝘁𝗲𝘀: By submitting this form, you acknowledge your responsibility for all payments due during the notice period and agree not to initiate chargebacks or payment disputes for these agreed-upon charges, subject to applicable law.

Once your cancellation request is submitted, you will receive a confirmation email outlining your final session dates and remaining billing schedule.
