Under the terms of this Agreement, I, {nameOf7},  hereby release, acquit and discharge any and all claims arising from or related to my child's entry and participation in the classes offered by Harbor House Cooperative or it's vendors.

By signing this document, I hereby grant permission for my child to participate in any and all class related activities on campus at 767 N Ventura Avenue. Participation in all events are voluntary. All participants, parents/guardians and family members of participants, acknowledge and agree that they will assume all risk associated with participation and waive any action, claim and any and all liability against Harbor House Cooperative and related vendors, Two Trees Community Inc., its officers, directors, agents, employees and volunteers, for any damage - including, but not limited to: injury or harm of any kind resulting from or occurring during participation, all obligations, pecuniary loss and intangible damages, the types of claims for personal injury, property damage, or any other Claims arising from or related to the undersigned’s entry into and participation in the activities. I assume full responsibility for any and all liabilities, losses, damages, costs, or expenses of any kind incurred by any reason to any person or property caused by my child.
