Declaration of Legal Guardianship, Waiver and Assumption of Risk and IndemnityTo: All directors, agents and personnel of Caribou Boys Camp (all of whom are herein referred to as “The Caribou Boys Camp representatives”) I, _______________________, Parent or Legal Guardian of _______________________(herein referred to as the Minor) hereby consent to the Minor’s participation in the Caribou Boys Camp from August 23-28th, 2026, in the province of Ontario, and hereby entrust the Minor to the guardianship and care of the Caribou Boys Camp representatives for the period from August 23-28th, 2026. I hereby authorize the said Caribou Boys Camp representatives to consent to any emergency medical treatment that the Minor may require during this period. I am aware that participation in the camp, including transportation to and from the camp, can present risks and dangers to the life and health of the Minor, and I hereby freely accept and assume the same entirely at my own risk. Consequently, I hereby waive any and all claims which the Minor or I may have against the Caribou Boys Camp representatives and release the Caribou Boys Camp representatives from all liability for injury, loss of life, property damage or other loss sustained by the Minor or me as a result of the Minor’s participation in Caribou Boys Camp, due to any cause whatsoever. I appreciate that this agreement applies whether the Caribou Boys Camp representatives are at fault or not. I also appreciate and accept that the Minor may be sent home from the camp should his behaviour prove disruptive or otherwise unacceptable. In this case, the Caribou Boys Camp representatives will communicate with me so that together we can determine the most suitable means of transportation, and I undertake to assume the full cost of said transportation. I hereby sign the present agreement on behalf of myself, my personal representatives, heirs and assigns. *
