I confirm that I have never been investigated, charged, or convicted of any form of animal cruelty, neglect, or negligence; child abuse or endangerment; domestic violence; or any other criminal offense that would call into question my suitability to work with vulnerable populations or animals. I understand that providing false information or failing to disclose relevant history may result in disqualification or dismissal from the AASR volunteer program.

As a volunteer with All Animals Sanctuary and Rescue (AASR), I understand and agree to conduct myself in a manner that ensures the safety and well-being of all individuals, including minors, staff, fellow volunteers, and animals. I acknowledge that AASR maintains a zero-tolerance policy regarding sexual abuse, harassment, or misconduct of any kind. I agree to uphold the organization's standards of conduct and understand that any violation of these policies may result in immediate termination of volunteer involvement and possible legal action.

I understand that working with animals carries inherent risks, including the possibility of injury to myself or others. By volunteering with All Animals Sanctuary and Rescue (AASR), I agree to take full responsibility for any animal I choose to handle or allow others to handle under my supervision. I voluntarily assume all risks associated with animal interactions and agree to hold harmless and indemnify AASR, its directors, staff, and affiliates from any claims, damages, or liabilities arising from such interactions.

If I choose to use my personal vehicle to transport animals, supplies, or perform any tasks on behalf of All Animals Sanctuary and Rescue (AASR), I do so voluntarily and at my own risk. I understand that I am fully responsible for my vehicle, any passengers, and any incidents that may occur during the course of such activities. I agree to hold harmless and indemnify AASR, its directors, staff, and affiliates from any claims, damages, or liabilities resulting from the use of my personal vehicle in connection with volunteer work.

By sending this electronically, I acknowledge that I have completely read this questionnaire and comprehend it fully. I understand that while volunteering at AASR I am to maintain the utmost standards of animal welfare and ethics while caring for any animal including thier social, emotional and physical wellbeing.

I understand that applying does not ensure approval for volunteer and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any All Animals Sanctuary and Rescue volunteering.

I understand that by submitting this form electronically, I agree to release and covenant to hold harmless All Animals Sanctuary and Rescue and it's members from any claims, damages, costs, or actions incurred because of the care or actions of any animal(s).

I accept full responsibility for the animal(s) actions at all times, and release All Animals Sanctuary and Rescue from any liabilities or damages that may be incurred because of volunteering with such animal(s).

I agree to have All Animals Sanctuary and Rescue complete reference call checks and conduct a home visit inspection to be able to approve my volunteer application if needed.

I agree to hold harmless All Animals Sanctuary and Rescue, Kiersten Sledge and/or Marcus Sledge and keep the tenants to this agreement for any and all animals that I volunteer with in 2025.

Minor Volunteer Release & Parental/Guardian ConsentI, the undersigned parent or legal guardian of the minor named below, give permission for my child to volunteer with All Animals Sanctuary and Rescue (AASR). I understand that my child may be working around animals and engaging in activities that carry inherent risks. I acknowledge and accept these risks and agree to hold harmless and indemnify AASR, its directors, staff, volunteers, and affiliates from any claims, damages, or liabilities that may arise from my child’s participation. I confirm that my child is physically and emotionally capable of safely handling animals and participating in volunteer activities. I understand that my child must follow all safety rules and instructions provided by AASR staff and volunteers.
