The primary contact* and any additional agency contacts will access the 2025 Application utilizing the same login information as used previously. UW staff will provide access to the 2025 Application within 1-2 business days, excluding weekends. This is not an automatic process, but will occur within the timeline outlined above. If you do not see the 2025 Application on your participant profile after two business days, please contact ella.weaver@yourlocaluw.org. Please read these Applicant Instructions prior to logging in and beginning your application!

Following submission of this form, UW staff will provide access to all requested program/agency contacts within 1-2 business days, excluding weekends. After your account has been created, you will receive an autoamted email with login instructions, including a temporary password. The automated email will not come immediately after completing the form, but will be sent within the timeline outlined above.
