PRIVATE EVENT REQUEST FORM

Wanting to host a private event with permanent jewelry? You're in the right spot! Please complete the following form, with as much information as possible.
PLEASE NOTE: This form does not confirm your private party. This is simply a place for you to give me details about your party. Once I review your information, I will reach out to you on the phone to chat about your event and make sure we're a good fit. Though I do not charge for a private party, I do require a credit card on file. If you want to move forward at that point, I will collect your credit card information then, on the phone. Only when I have your credit card on file, will you be confirmed, and your date be blocked from my calendar.

Please review the following terms and conditions, and initial your consent next to each.
MINIMUM PURCHASE: To host a private event, there is a minimum requirement of seven (7) sales, each valued at $70 or more, OR a total sales amount of at least $490. If this minimum is not met, the credit card on file will be charged for the difference in order to reach the required minimum of $490 ($70 x 7 sales). By submitting this request, you acknowledge and agree to these terms. If attendees are paying for their own pieces, please ensure that they are aware of this policy and ready to purchase their own product(s). I take cash, check, apple pay, google pay, cash app or any major credit card. *
CANCELLATION POLICY: If you need to cancel, please do so at your earliest convenience so that I can open the scheduled date on my calendar again. Cancellations made within 48 hours of the event will result in a $490 charge to the credit card on file. (***Private parties in November & December have a 7 day cancellation policy as these dates are in higher demand / the dates are less likely to be filled once cancelled)*
