    The Diagnosis of Personality Disorder
  • Thank you for considering sharing your experiences with Platfform through the Truth Project. We’re aiming to highlight the lived reality of being given a diagnosis of personality disorder. Whether that is your own experience, working alongside people, or family members or friends, we want to capture a range of experiences. You can find out more about how we use people’s stories to drive change here

    Some details about you

  • Why we ask for your location

We ask which location of the UK your experiences took place in to help us better inform our reports and help make the case for the things that need to change.

You do not need to share the region, but if you do, it will help Platfform to understand what is happening in what areas. This then helps us focus our efforts where they are needed the most. Your location will not be made public.

  • Sharing your story

We understand that sharing personal information about difficult experiences could be emotionally distressing, so you may want to let a trusted friend know or talk to someone who supports you, before sharing your full story with us.

If we publish your story, we will always try and use your own words as much as possible. We may remove personal details for you or others mentioned, and we may make other small changes to meet safety guidelines.

  • Part 3

What you would like us to do with your story?
  • We will ask you here if you would like your real name or alternative name to be attached to your story. Please consider this option carefully. If it is unclear to us, we will always default to the alternative name you provide.

You can of course choose to remain completely anonymous, even if you have provided your name.

  • Creative Story Telling

We believe that telling your story in a creative way like film, painting, craft, exhibition, theatre, spoken word, poetry, podcasting, music or any other artform can be a powerful way to share difficult stories with a wider audience. This is something Platfform is currently exploring alongside people with lived experience of the diagnoses.

We would love to know if you would be interested in joining this side of the campaign work. You don’t need to be creative to be involved; we’re just interested to know if you’d like to explore this further with us and maybe even learn some new skills along the way.

By answering the questions below, you’ll help us understand how we may collaborate with you in the future.

If you would like us to contact you about any of this, please make sure you have provided contact information in Section 1 of this form
