This form with reserve dates requested is based on space availability. Team Events will not necessarily be bowled ahead of doubles and singles. If you do not designate estimated number of doubles and singles, they will NOT be reserved. *Bowlers must bowl both doubles and singles*. If two teams wish to bowl together – entry forms must be submitted together. Official entry blanks will be mailed with confirmations. This form will be the only accepted form for reservation request.
