  • ---PLEASE KNOW--- completing this form does NOT guarantee registration for your child!  We will need to confirm availability for each registration.  Once we confirm there is a slot available, you will receive an email from the Day School.  Then, you will be asked to send in the required $125 nonrefundable registration fee.

    Once we confirm the spot and receive the fee, your registration will be confirmed. 

    You may stop by our office to pay the registration fee or mail in a check made out to FBCDS at:

    After one week, if FBCDS has not received your Registration Fee, we will send you a reminder email.  If we get no response, we will contact you directly. 

    Note: You will need to submit a form and registration fee for each child you are registering.

  • I understand a LATE TUITION FEE will be imposed on me at the rate of $15 if my tuition payment is not made by the 15th of each month*
  • I understand a LATE PICK-UP FEE will be imposed on me at the rate of $1.00 PER MINUTE if my child is picked up after dismissal time (12:30 PM regularly or 1:30 PM after Lunch Bunch)*
