Required Waivers and Forms

We keep some standard over-the-counter medicine(s) on hand at Ignite for headaches, or other common ailments. (tylenol, advil, benedryl, tums, hydrocortisone, etc). Please indicate below whether or not you give your permission to the staff at Ignite to provide these to your student if needed.  Space has been given for up to 5 children.  Please list each student indiviudally and provide permissions for each.

If your child needs to regularly take a prescription medication while at Ignite, please send that medication in with their name, dosage, and the time that they need to take it.  We will keep this in the office, and give it to your child when needed.

Notice: No medication may be used or possessed at Ignite unless the Leadership Team receives this completed form. All medicine brought into Ignite must be kept in the office and must be in the original container, appropriately labeled by the pharmacy/ physician or in the bottle it was purchased in.

Parental Consent and Waiver
I hereby give my permission for my child to take the above medication at Ignite.

Waiver of Liability
I understand that Ignite Leadership will administer only the medication mentioned above. I hereby waive any and all claims against Ignite and/or the Board, and agree to hold Ignite harmless from any and all liability, which may arise in connection with my child’s use of the medication.

As a  parent or guardian of this student, I hereby grant Ignite Co-op permission to use my or my child’s photographs and/or digital images/videos publically to promote the Co-op. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS EVIDENCED BY THEIR SIGNATURES BELOW.

If you wish for your student to have permission to leave campus during the school day, Ignite will have to have this form signed and on file in the office.  Please read through each section carefully. Please check all statements that apply. Students will only have the permission granted to them on this form.  If you need to make an exception, we must receive a written statement from the parent or guardian.  If your student will be driving at any time during the school year, you will also need to sign a “Student Driver Guidelines and Waiver” form. 

Ignite Homeschool Co-op is not responsible for students when they are not on campus.  

Students must sign in and out at the office if they leave campus during the school day.

Students must be back on campus at least 5 minutes before the beginning of their next class. 

Students that have received a lunch detention will not be allowed to leave campus during lunch on that day.

YOUTH WAIVER AND RELEASE OF LIABILITY 

In consideration of being allowed to participate in any way in  Ignite Homeschool Co-op related events and activities, the undersigned: 

1. Agree that the parent(s) and/or legal guardian(s) of the participant should inspect the facilities and equipment to be used, and if the parent or guardian believes anything is unsafe, he or she should immediately advise supervisor (advisor, manager, etc.) of such condition(s) and refuse to participate. 

2. Acknowledge and fully understand that each member/participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence but the action, inaction, and negligence of others, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time. 

3. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death. 

4. Release, waive, discharge and covenant not to sue Ignite Homeschool Co-op, its affiliated clubs, their respective administrators, directors, agents, and other employees of the organization, other members/participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors or premises used to conduct the event, all of which are hereinafter referred to as “releases,” from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death and damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.

ADULT WAIVER AND RELEASE OF LIABILITY 

In consideration of being allowed to participate in any way in Ignite Homeschool Co-op 

related events and activities, the undersigned:

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing, the wearing of masks, and other such precautions.

Ignite Homeschool Co-op and Siloam Baptist Church have put in place preventative measures to reduce the spread of COVID-19; however, we cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending Ignite could increase your risk and your child(ren)’s risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Ignite and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Ignite may result from the actions, omissions, or negligence of myself and others, including, but not limited to, teachers, staff, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at Ignite.  On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Ignite Homeschool Co-op, Siloam Baptist Church, their employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Ignite Homeschool Co-op or Siloam Baptist Church, their employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Ignite program.

My student has permission to drive to Ignite Homeschool Co-op and agrees to adhere to the following guidelines while on the Ignite campus. We understand that it is a privilege for our student to drive on campus and the privilege can be revoked if the Ignite Leadership Team finds that my student is not following or adhering to the agreed upon guidelines.

 

My student is aware that they need to park in the Ignite designated parking lot only.
Reckless driving in the parking lot will result in the loss of parking privileges for the remainder of the year.
Drivers and passengers in student vehicles may not loiter in the parking lot before or after school.
If written permission is given to leave campus during the school day before the student driver’s last class, no other student will be allowed to leave with them without advance written permission from both parents.
Students may not return to their vehicles during the school day without approval from a member of the Ignite Leadership Team.
Students are reminded to use common sense with regard to securing vehicles. Vehicles should be locked with valuables placed out of sight. Neither Ignite Homeschool Co-op or Siloam Baptist Church or Carolina Heights will be responsible for any theft or vandalism that occurs to vehicles parked on church property.
Students leaving for any reason during school hours must sign out with the Office and sign back in when they return.
Students are aware that no other student will be allowed to leave with them without advance written permission from both parents.

In consideration of being permitted to participate in The Ignite Homeschool Coop, 

Release, waive, discharge, and covenant not to sue either the Ignite Homeschool Co-op or the host facility, their officers, employees, board members or other families participating in the Ignite Homeschool Co-op (hereinafter referred to as ‘Releasees’) from all liability to Family for any loss or damage and any claim or damage on account of injury to the person or property or resulting in death of a Family member while Family is participating in the Ignite Homeschool Co-op activities or any other activities on  or off the premises of the host facility.  

Family agrees to indemnify Releasees from any loss, liability, damage or cost Releasees may incur due to the presence of Family in or on the premises of the host facility. Family realizes transportation to and from Co-op or any time off campus is the Family responsibility.     

Family assumes full responsibility for and risk of bodily injury, death or property damage while in or on the premises of the host facility and /or while working or for any purpose participating in the activities of the Ignite Academy Co-op.    

Family expressly agrees that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the State of South Carolina and that, if any portion of the agreement is held invalid, it is agreed that the balance, shall notwithstanding, continue in full legal force and effect.     
 

Family, in consideration of being permitted to participate in the Ignite Homeschool Co-op, for Family, and Family’s heirs, executors, administrators, and assigns, releases and forever discharges all Releasees, and their heirs, administrators, and executors of and from any and every claim, demand, action or right of action, or whatsoever kind or nature, either in law or in equity arising from or by reason of any bodily injury or personal injuries known or unknown, death or property damage resulting or to result from any accident which may occur as a result of participation in the Ignite Homeschool Co-op or any activities in connection with the Ignite Homeschool Co-op.  

Family states that both mother and father of Family or legal guardians have carefully read the above release and know the contents of the release and sign this release as Family’s own free act. 
 

Family releases all officials and professional personnel from any claims whatsoever on account of first aid, treatment or service rendered Family during participation in the Ignite Homeschool Co-op.     
 

The Ignite Homeschool Co-op operates under Biblical guidelines and believes that the Bible commands us to make every effort to live at peace and to resolve disputes with each other in private or within the Christian church (see Matthew 18:15-20; 1 Corinthians 6:1-8). Therefore, the parties agree that any claim or dispute arising from or related to this agreement shall be settled by biblically based mediation. The parties understand that these methods shall be the sole remedy for any controversy or claim arising out of this agreement or any dispute arising out of any activities or events associated with the Ignite Homeschool Co-op and expressly waive their right to file a lawsuit in any civil court against one another or against another Ignite Homeschool Co-op family for such disputes, except to enforce an arbitration decision. 

This agreement shall be binding on each member of the Family, their personal representatives, assigns, heirs, and next of kin.   

This release contains the entire agreement between the parties to this agreement and the terms of this release are contractual and not a mere recital.
