Consent and Waiver of Liability
By signing below, I acknowledge that I am voluntarily participating in physical activities organized by The House of
Faith (hereinafter referred to as “the Organization”). I understand and agree to the following:
1. Assumption of Risk: I am aware that participating in physical activities involves inherent risks, including but not
limited to personal injury, property damage, or death. I voluntarily assume all such risks, both known and
unknown, even if arising from the negligence of the Organization or others.
2. Physical Fitness: I affirm that I am in good health and physically capable of participating in the activities. I will
inform the Organization of any medical conditions that may affect my participation.
3. Waiver of Liability: To the fullest extent permitted by law, I waive any right to hold the Organization, its officers,
directors, employees, volunteers, or agents liable for any injury, loss, or damage sustained during or as a result of
my participation in these activities.
4. Indemnification: I agree to indemnify and hold harmless the Organization from any claims, actions, suits,
procedures, costs, expenses, damages, and liabilities, including attorney’s fees, arising out of my involvement in
these activities.
5. Emergency Treatment: In the event of an injury or medical emergency, I authorize the Organization to seek
medical treatment on my behalf if I am unable to do so myself.
6. Compliance with Rules: I agree to comply with all rules, regulations, and instructions provided by the
Organization or its representatives during the activities.
7. Media Release: I grant permission for the Organization to use my likeness in photographs, videos, or other media
for promotional purposes without compensation.
8. Severability: If any portion of this waiver is deemed unenforceable, the remainder shall remain in full force and
effect.
