In order to finalize the application, please initial and sign the program agreement
Registration fee of $250 per family (non-refundable/non-transferable)*
I realize that no refund of fees or credits will be made for withdrawal or absences.*
Any changes/withdrawals must be requested in writing.*
I understand that a membership to Saw Mill Club or Saw Mill Club East is REQUIRED*
If necessary, I authorize the persons in charge to administer or secure first aid emergency medical treatment for my child.*
I authorize my child to be photographed for the Preschool Brightwheel App.
I authorize my child to be photographed for Genesis Saw Mill promotional purposes, social media and/or website.
I hereby give consent for my child to participate in the Preschool program at Genesis-Saw Mill Club. Saw Mill Preschool admits students of any race, religion, ethnic origin, gender or disabilities.
