Vaccination and Parasite Control Policy:
To prevent the spread of infectious diseases and parasites, all hospitalized and boarded animals must be up to date on vaccinations and free of internal and external parasites. By signing below, I authorize the veterinarian to provide necessary vaccinations and parasite control for my pet at my expense. Upon request, estimates for any services will be provided. Professional fees are due at the time services are rendered. For hospitalization or boarding, a 70% deposit is required at admission, with the remaining balance due at discharge.
I understand that if I do not claim my pet within ten (10) days of the scheduled discharge date, my pet will be considered abandoned, and legal action, including rehoming, may be taken. I accept full financial responsibility for all incurred fees, including but not limited to veterinary charges, legal fees, court costs, and collection fees.
I hereby authorize the veterinarian to examine, prescribe for, or treat the described pet. I accept full responsibility for all charges incurred during the care of this animal. I understand that these charges are payable at the time of release and that a deposit may be required for surgical procedures. Any unpaid balance exceeding 30 days may be processed through Care Credit, pending approval.
Para prevenir la propagación de enfermedades infecciosas y parásitos, todos los animales hospitalizados o alojados deben estar al día con sus vacunas y libres de parásitos internos y externos. Autorizo al veterinario a proporcionar las vacunas necesarias y el control de parásitos para mi mascota a mi cargo. A solicitud, se proporcionarán estimados para cualquier servicio. Las tarifas profesionales deben pagarse en el momento en que se prestan los servicios. Media Release:
I hereby grant and authorize Animal Hospital of Pembroke Road the right to take, publish, and make use of any pictures or videos taken of me and my pet(s) by the clinic staff. This authorization extends to all languages, media, and forms of marketing now known or hereafter devised. This authorization shall continue indefinitely unless revoked in writing.

Waiver of Responsibility:
I understand and agree that the Animal Hospital of Pembroke Road, its employees, and representatives are not liable for any injury, loss, or damage to me, my pet(s), or my belongings while on the premises or during the course of veterinary care. This includes, but is not limited to, incidents that may arise from interactions with other clients, pets, or the facility itself.
