Liability Waiver
I, the undersigned parent or legal guardian of the child(ren) named above, acknowledge and agree to the following terms and conditions regarding my child's participation in the childcare services provided by Motor City Sitters at the event organized by Semester in Detroit, The Bogg School, and the University of Michigan:

Assumption of Risk: I understand and acknowledge that participation in childcare services carries inherent risks, including but not limited to minor injuries, illnesses, and property damage. I voluntarily assume all risks associated with my child's participation in the childcare services provided during this event.

Release of Liability: I hereby release, waive, and discharge Motor City Sitters, its caregivers, Detroiters' Voices, The Bogg School, the University of Michigan, and any event organizers, affiliates, employees, agents, or representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness that may occur while my child is under the care of the childcare providers at this event.

Medical Authorization: In the event of an emergency, I authorize the caregivers to administer basic first aid and/or seek emergency medical treatment for my child if deemed necessary. I understand that every effort will be made to contact me or my designated emergency contact immediately.

Behavior and Misuse of Space: I understand that I am responsible for my child's behavior and actions while in the childcare space. If my child engages in behavior that is disruptive, unsafe, or results in damage to the venue or event space, I accept full responsibility for any resulting consequences or damages.

Indemnification: I agree to indemnify and hold harmless Motor City Sitters, its caregivers, Detroiters' Voices, The Bogg School, the University of Michigan, and event organizers from any claims, liabilities, damages, or expenses arising from my child's actions or participation in the childcare services.

Acknowledgment and Understanding: I have carefully read and fully understand this waiver and release of liability. I voluntarily agree to its terms and acknowledge that by signing this document, I am waiving certain legal rights.
