I hereby grant and authorize Animal Hospital of Pembroke Road the right to take, publish, and make use of any pictures or videos taken of me and my pet(s) by the clinic staff. This authorization extends to all languages, media, and forms of marketing now known or hereafter devised. This authorization shall continue indefinitely unless revoked in writing.

Waiver of Responsibility:
I understand and agree that the Animal Hospital of Pembroke Road, its employees, and representatives are not liable for any injury, loss, or damage to me, my pet(s), or my belongings while on the premises or during the course of veterinary care. This includes, but is not limited to, incidents that may arise from interactions with other clients, pets, or the facility itself.
