STUDENT PERMISSION FORM
The Quantum Kids Education and its schools carry out ongoing marketing and communication activities to maintaina positive profile in their local communities.
These initiatives usually involve images of students, families and individuals being utilised in all forms of media – print,electronic, online and social media . Common methods of publication have included brochures, newsletters,newspaper, radio and television advertisements, promotional DVDs, web sites and educational training (photographsand/or footage). Some promotional images may also be used on the Quantum Kids Education and/or individualschools social media pages including Facebook, Instagram and other social media platform.
To comply with our organisation’s privacy obligations and in keeping with applicable legislation, it is a requirementthat consent be given for such activities by either the individual (if over 18 years) or by a parent/guardian (if under 18
years).Consent will be sought from the parent, guardian or individual (if over 18 years of age) by asking them to completeand sign this form. The consent form will be retained by the Schools Office or the individual school, depending on the
purpose. NB: This consent, once given, will be amended or revoked only upon receipt of correspondence from theparent/guardian or individual (if over 18 years).
I authorise Quantum Kids Education and/or its approved education partners to take and use any photographs, video or sound recording of me/the student and any reproductions or adaptations of the material, either in full or part, in
conjunction with any wording or drawings, in any Quantum Kids Education publication, production and presentation.This includes use on the internet and social media platforms such as Facebook, Instagram and other social media
platform.I acknowledge that I/the student has no rights in the material in any Quantum Kids Education publication, productionor presentation which includes the material.
Agreement
Enroling your child in Quantum Kids Education means that you are choosing an online homeschoolingapproach tailored to support each student’s academic growth, independence, and readiness for lifelong
learning. This agreement outlines key responsibilities and expectations for both students and parents toensure a productive and engaging learning experience. By enrolling, you agree to the following terms:

1.Academic Commitment

You understand that Quantum Kids Education provides a structured, comprehensive onlinecurriculum designed to meet academic standards.Your child is expected to engage in course work, complete assignments on time, and participate in assessments. We request that students are able to been seen so that engagment is meaningful and colloborative.

2.Standards of Conduct

Your child is expected to adhere to the school’s standards, including:
Appropriate behavior during online sessions and respect towards classmates and teachers.Timely attendance and active participation in scheduled online classes.
Observing the school’s policies on academic integrity and respectful communication.

3.Parental Involvement

Parents are encouraged to actively support their child’s learning by:
Monitoring their progress and ensuring they complete assignments and participate in online
activities.Attending scheduled virtual parent-teacher meetings to discuss academic and developmentalprogress.Staying informed about school updates, policies, and announcements.

4.Information & Communication Technology (ICT) Policies

We expect adherence to Quantum Kids Education’s ICT policies, which includes appropriate use ofthe internet and school-provided online resources. Parents should review these guidelines withtheir child.Any misuse of technology or inappropriate online behavior may lead to disciplinary action in linewith the school’s ICT policies.

5.Emergency and Health Provisions

In case of emergencies, health issues, or accidents, Quantum Kids Education requires parents to provide up-to-date contact information and any relevant medical information. While we are not responsible for medical care, we may guide parents to appropriate resources if necessary.

6.Data Privacy and Information Accuracy

You agree to keep all personal and medical information provided to Quantum Kids Educationaccurate and up to date. This includes notifying the school of any changes in contact details,health concerns, or other relevant information that may impact your child’s learning experience.

7.Participation in School Online Activities

Students are expected to participate in both academic and extracurricular activities as scheduledby Quantum Kids Education to enrich their learning journey and foster a sense of community.

8.Fee Agreement

You agree to honor the financial commitments required by Quantum Kids Education, includingtimely payment of tuition fees as per the Schedule of Fees and Charges.

9.Declaration of Understanding

I/We have read all of the information in the enrolment package and understand the policies that must be followed for successful enrolment in Quantum Kids Education. We acknowledge that any misleading information or omission of significant, relevant details provided during enrolment mayresult in the withdrawal of the enrolment.

By enrolling your child in Quantum Live Classes, you acknowledge and agree to the following terms and conditions:1. Non-Refundable PolicyAll enrolment fees, class payments, and tuition charges for Quantum Live Classes are strictly non-refundable once payment has been made, regardless of the reason for cancellation, withdrawal, absence, or non-attendance.This applies even if the student does not attend the scheduled classes after enrolment.2. Class Access and ParticipationUpon successful enrolment and payment, your child will be granted access to live online classes, mentoring sessions, and learning materials according to the schedule provided.It is the parent’s or guardian’s responsibility to ensure the student attends classes on time and has the required device, internet connection, and learning materials.3. Payment Obligations and SuspensionAll invoices must be paid on or before their due date.Failure to pay your invoice may result in the temporary suspension or removal of your child from live classes and access to learning resources until the outstanding balance is fully paid.Quantum Kids Education reserves the right to charge late fees or pursue recovery of unpaid amounts in accordance with applicable laws.4. Schedule Changes and CancellationsQuantum Kids Education reserves the right to modify class schedules, teachers, or session times when necessary. In such cases, reasonable notice will be provided, and alternative sessions or arrangements will be offered.5. Attendance and Missed ClassesMissed classes due to student absence will not be refunded or credited.Where possible, make-up classes or recordings may be offered at the discretion of Quantum Kids Education but are not guaranteed.6. Code of ConductStudents are expected to maintain respectful and appropriate behaviour during classes. Quantum Kids Education reserves the right to suspend or remove any student who disrupts classes or violates community standards.7. Acceptance of TermsBy completing this enrolment, you confirm that you have read, understood, and agreed to these Terms and Conditions — including the non-refundable policy and the suspension of services in the event of non-payment.
