If you need an explanation of any of the questionsor help in completing this application, please ask for assistance from the school staff. You are welcome to provide further information on an attached sheet. An explanation of the purpose and use of the information you provide is given on page 9.The school will notify you of the results of your application. The information you have provided will be used by the school for student enrolment if your application is accepted.

STUDENT PERMISSION FORM
The Quantum Kids Education and its schools carry out ongoing marketing and communication activities to maintaina positive profile in their local communities.
These initiatives usually involve images of students, families and individuals being utilised in all forms of media – print,electronic, online and social media . Common methods of publication have included brochures, newsletters,newspaper, radio and television advertisements, promotional DVDs, web sites and educational training (photographsand/or footage). Some promotional images may also be used on the Quantum Kids Education and/or individualschools social media pages including Facebook, Instagram and other social media platform.
To comply with our organisation’s privacy obligations and in keeping with applicable legislation, it is a requirementthat consent be given for such activities by either the individual (if over 18 years) or by a parent/guardian (if under 18
years).Consent will be sought from the parent, guardian or individual (if over 18 years of age) by asking them to completeand sign this form. The consent form will be retained by the Schools Office or the individual school, depending on the
purpose. NB: This consent, once given, will be amended or revoked only upon receipt of correspondence from theparent/guardian or individual (if over 18 years).
I authorise Quantum Kids Education and/or its approved education partners to take and use any photographs, videoor sound recording of me/the student and any reproductions or adaptations of the material, either in full or part, in
conjunction with any wording or drawings, in any Quantum Kids Education publication, production and presentation.This includes use on the internet and social media platforms such as Facebook, Instagram and other social media
platform.I acknowledge that I/the student has no rights in the material in any Quantum Kids Education publication, productionor presentation which includes the material.

Agreement
Enroling your child in Quantum Kids Education means that you are choosing an online homeschoolingapproach tailored to support each student’s academic growth, independence, and readiness for lifelong
learning. This agreement outlines key responsibilities and expectations for both students and parents toensure a productive and engaging learning experience. By enrolling, you agree to the following terms:

1.Academic Commitment

You understand that Quantum Kids Education provides a structured, comprehensive onlinecurriculum designed to meet academic standards.Your child is expected to engage in course work, complete assignments on time, and participate in assessments.

2.Standards of Conduct

Your child is expected to adhere to the school’s standards, including:
Appropriate behavior during online sessions and respect towards classmates and teachers.Timely attendance and active participation in scheduled online classes.
Observing the school’s policies on academic integrity and respectful communication.

3.Parental Involvement

Parents are encouraged to actively support their child’s learning by:
Monitoring their progress and ensuring they complete assignments and participate in online
activities.Attending scheduled virtual parent-teacher meetings to discuss academic and developmentalprogress.Staying informed about school updates, policies, and announcements.

4.Information & Communication Technology (ICT) Policies

We expect adherence to Quantum Kids Education’s ICT policies, which includes appropriate use ofthe internet and school-provided online resources. Parents should review these guidelines withtheir child.Any misuse of technology or inappropriate online behavior may lead to disciplinary action in linewith the school’s ICT policies.

5.Emergency and Health Provisions

In case of emergencies, health issues, or accidents, Quantum Kids Education requires parents toprovide up-to-date contact information and any relevant medical information. While we are not responsible for medical care, we may guide parents to appropriate resources if necessary.

6.Data Privacy and Information Accuracy

You agree to keep all personal and medical information provided to Quantum Kids Educationaccurate and up to date. This includes notifying the school of any changes in contact details,health concerns, or other relevant information that may impact your child’s learning experience.

7.Participation in School Online Activities

Students are expected to participate in both academic and extracurricular activities as scheduledby Quantum Kids Education to enrich their learning journey and foster a sense of community.

8.Fee Agreement

You agree to honor the financial commitments required by Quantum Kids Education, includingtimely payment of tuition fees as per the Schedule of Fees and Charges.

9.Declaration of Understanding

I/We have read all of the information in the enrolment package and understand the policies that must be followed for successful enrolment in Quantum Kids Education. We acknowledge that any misleading information or omission of significant, relevant details provided during enrolment mayresult in the withdrawal of the enrolment.

Agreement Confirmation By enrolling your child in Quantum Kids Education, you acknowledge that you have read and agree to the terms outlined above.

What is this consent for?
This Consent Form authorises Quantum Kids Education to use the Individual’s copyright material, image and recording. The consent covers the entire or
partial use of the Individual’s copyright material, image, and recording in conjunction with other words and images.
What is an image or recording?
In this Consent Form, an image or recording includes photographs, videos, films, or sound recordings of the individual.

Personal information, consent, and declaration of accuracy

The personal information collected on this application is
for purposes directly related to your child’s education
including processing this application.
Any information provided to the Quantum Kids Education
will be used, disclosed and stored consistent with the NSW
privacy laws.
Certain information is required by the Department to meet
its duty of care and other legal obligations under public
health, education and child protection legislation and for
meeting data collection and reporting requirements under
Commonwealth – State funding agreements which may
involve evaluation and assessment of student outcomes.
Information may be disclosed to the State and
Commonwealth government agencies and other
organisations for the above purposes and as authorised or
required by law.
Information will be stored on a secure electronic database.
You may access or correct the information by contacting
your child’s school. If you have a concern or complaint
about the information collected or how it has been used or
disclosed you should contact the school.
If you choose not to provide some requested information it
may have a detrimental impact on your child’s enrolment,
resourcing of the school or meeting your child’s
educational needs
Further information about the collection of information
while your child is enrolled at Quantum Kids Education,
and how we protect your privacy, is available on the
School website or from your school.

Publishing student information
The school/Department may publish information about
your child for the purposes of sharing his/her experiences
with other students, informing the school and broader
community about school and student activities and
recording student participation in noteworthy projects or
community service.
This information may include your child’s name, age, class
and information collected at school such as photographs,
sound and visual recordings of your child, your child’s work
and expressions of opinion such as in interactive media.
The communications in which your child’s information
may be published include but are not limited to:
Public websites of the Department including the school
website, the Department’s intranet (staff only), blogs
and wikis.
School publications including the school newsletter,
annual school magazine and school report,
promotional material published in print and
electronically including on the school websites.
Official school social media accounts on networks
such as the school’s YouTube, Facebook and Twitter
pages.Parents should be aware that when information is published
on public websites and social media channels it can be
linked to by third parties and may be discoverable online for
a number of years, if not permanently. Search engines may
also cache or retain copies of published information.

 

Online services
We do not provide the students with filtered internet access.It is the parent’s/guardian’s responsibility to monitor and filter internet usage to ensure safe and appropriate online behavior. Quantum Kids Education does not hold any
responsibility for misuse of the internet by the student. Misuse of the internet by the student can include, but is notlimited to: accessing inappropriate or harmful content,engaging in cyber bullying, sharing personal or sensitive information, unauthorized downloading or uploading of files,or any activity that violates laws, regulations, or ethical guidelines. Parents/guardians are encouraged to take an
active role in supervising their child’s online activities to mitigate these risks.
I give Permission and agree to the terms for my child to have access to online services. This permission remains effective until I advise the school otherwise.
Consent
I have provided information about the learning and support
needs, including health condition(s) and/or special need(s)
and/or history relevant to a risk assessment, related to the
student listed in Section A of this application form.
I consent to the school seeking information from previous
schools, other NSW government departments or other
organisations that may also hold information related to this
assessment for the student named on page 1.
