Mayapple Summer Camp Registration 2026

Welcome to The Mayapple School Summer Camp Registration form. To read more about each camp, go to: Mayapple School Summer Camps . A separate registration form is required for each child. 

This form may not work properly using your cell phone. Please use a laptop or desktop computer if you encounter any problems. 

Either:

• Pay online through this form. Paying online incurs a processing fee, which is included below.
• Remit payment by check or bank payment to: The Mayapple School 2895 Old Fort Rd. Blacksburg, VA 24060. Refer to the costs of the camp on our summer camp page, which avoids the processing fee. Write "paybycheck" in the discount box below.
• Please note that the costs below and the the costs on our website differ, as the costs below include the online payment processing fee. You can avoid the payment processing fee by paying by check and writing "paybycheck" in the discount box.
• Payment must be received within 3 weeks of camp sign up in order to continue to reserve your child's spot, except in the case of scholarship applications that fit the scholarship parameters as noted in the summer camp handbook.
  • For wrap-around care ONLY: Summer wrap around supply & registration payment of $100 is due upon registration. 1/2 of the fee is due within 3 weeks of camp sign up, and the remaining 1/2 is due June 15th. Alternate payment schedules are available for families using Child Care Subsidy - please inquire. 

Wrap-around and Summer Campers will need the following:

• School Entrance Health Form 
• Child Registration Form 
• Authorization Form for Sunscreen & Insect Repellent 

If your child has attended The Mayapple School as either a preschooler or a kinderforester in the last three years, you do not need to submit new forms. Simply request updated vaccination records from your Dr. and bring them to camp.

By signing below, you agree to follow the stipulations and requirements within the Summer Camp Parent Handbook, including the following:

• Should an emergency occur when the parent cannot be located immediately, emergency medical care is authorized.
• The center will notify the parent when the child becomes ill, and the parent will arrange to have the child picked up as soon as possible if requested by the center.
• The parent will inform the center within 24 hours or the next business day after their child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life-threatening diseases which must be reported immediately.
