SHOTS BY TY

I, the undersigned, hereby grant permission to Shots By Ty, an independent photographer, to take and use photographs and/or video of me or my child (if under the age of 18) during high school sporting events. I understand that these images may be used for promotional purposes, including but not limited to, marketing, social media, printed materials, and other forms of publicity related to SHOTS BY TY.

1. Permission to Use Images I grant permission for the images to be used in the following ways (check below which are permitted):

 

     Social Media (e.g., Instagram, Facebook, Twitter etc.

     Website/Blog

     Printed Materials (e.g., flyers, posters, etc.)

     Marketing and Promotional Materials

 

 

2. Ownership of Images Iacknowledge that Shots By Ty is the owner of the images and that I will not receive any compensation for their use unless otherwise specified in writing.

3. Consent to Editing Iunderstand that the images may be edited, altered, or modified in various ways as deemed necessary by the photographer or Shots By Ty.

4. No Obligation I understand that my decision to grant or not grant permission for the use of my images is completely voluntary. I am not required to participate in this project, and I can withdraw my consent at any time by submitting a written request to Shots By Ty.

If I am signing on behalf of a minor (under the age of 18), I hereby confirm that I am the parent or legal guardian of the minor and have the legal right to consent to the terms outlined in this form.

6. Liability I agree to hold harmless and release Shots By Ty, from any claims or damages related to the use of the images.
