Approval Process
By submitting this form you are attesting that this request complies with all checkout room rules, have a legitimate academic need for this equipment and can utilize all requested equipment without damaging it.
After you submit this request, it will route to the faculty member(s) you selected for review and approval. If they approve, it will route to the CoJMC checkout room manager for review and approval.
You will be notified by email of the outcome of the review. If approved, you will be required to present the approval email to the CoJMC Checkout Room to proceed with the checkout.
