According to NYS OCFS DAYCARE REGULATIONS:

A PHYSICAL DATED BETWEEN 9/1/2024-9/1/2025 MUST BE TURNED IN BY THE FIRST DAY OF SCHOOL.
- Please be aware of the dates listed above.
- Turning one in at registration, does not always count as an updated physical, because it is based on date.
- IF YOU DO NOT HAVE A CURRENT PHYSICAL by the first day of school, you may not be able to start UPK.
I have read and understand the rules for the current physicals and will have a current physical turned in by the first day.
