The undersigned, being parent, guardian, or legal custodian of the player/participant, hereby consents and allows the player/participant to participate in any and all activities of the SWBL Pee Wee Football League. The undersigned parent, guardian, or legal custodian acknowledges that the activities of the SWBL Pee Wee Football League includes the possible risk of physical injury to the participants and hereby assumes the risk of injury and releases the SWBL Pee Wee Football League, its agents or assigns and any and all coaches from any and all claims arising from the player/participant’s participation in the activities. I/We as parent(s), guardian(s), or legal custodian(s) of the player/participant hereby authorize the SWBL Pee Wee Football Legue, or agent thereof, to seek and employe whatever medical care and treatment that the SWBL Pee Wee Football League, or any agent thereof deems necessary in the event of an accident, injury, or illness of the player/participant on my our behalf and agree to pay for said care and/or treatment. The undersigned, being the parent, guardian or legal custodian of the player/participant hereby consents and agrees to the SWBL Playing Rules, the SWBL Constitution and SWBL Rules of Conduct.
