Dog Adoption Application

Paw Squad 559 is an all-volunteer, non-profit, animal welfare organization dedicated to the rescue and placement of stray, neglected and abused animals. Paw Squad 559 is completely funded through fundraising efforts and public donations. All of our pets are cared for by foster homes and receive complete medical care, which includes worming, vaccinations, spaying/neutering and microchipping prior to their adoption.

Your adoption fee allows us to continue our rescue efforts. A full refund of the adoption fee shall be refunded if the dog is taken to the vet and information is provided to Paw Squad 559 in writing from a licensed vet within 3 days of the adoption stating that the adopted animal has a life threatening medical condition. Paw Squad 559 will not be liable for any vet bills incurred. After that period, the adoption fee is NONREFUNDABLE. Understand that Paw Squad 559 has the right to approve or deny your adoption application without explaination. Adoption fees are paid at the time of the adoption. Incomplete applications will be rejected.
