PLEASE USE THIS FORM TO SUBMIT AN AMENDED OR NEW EMPLOYER PARTICIPATION AGREEMENT FOR THE UU ORG RETIREMENT PLAN: First, please select whether you are submitting an amended agreement or adopting as a new employer. Next, complete and review all cream colored fields (above the bold line).
If you are a current participating employer in the Plan submitting an amended Employer Participation Agreement, you will select your organization from the "UU Org Lookup" dropdown field. Several of the fields will then auto-populate with your organization's information. Continue scrolling down to see the information as it will appear on the agreement. If any information appears incorrectly on the agreement, please reach out to retirementplan@uua.org to request a correction before submitting your agreement.
Do not complete any initials/signature fields designated for the Board Treasurer (green) and President/Chair (red) unless you are that person.
When completed, press the green "Submit" button at the bottom. You should then see a Thank You message and your submission will be forward to the UUA for initial review and will then be routed to the treasurer and president to the emails provided for their e-initials and e-signature.
PRESIDENTS AND TREASURERS: If you are not the original submitter, after receiving your email inviting you to sign the agreement, please review all organizational data and elections as submitted and as they appear on the agreement below. If you find any discrepancies, please reach out to retirementplan@uua.org so that any necessary corrections can be made. Read each section and then add your initials and signature where indicated, Board Treasurer (green) and President/Chair (red). You can either type or draw your initials and signature in the respective boxes by selecting "type" or "draw". When completed, press the "Submit" button at the bottom. You should then see a Thank You message.
