Insurance Waiver Form
It is the responsibility of the camper's parent or guardian to ensure that the camper is healthy and has no physical problems that would prevent his participation in all camp activities and further, has had a physical within the last 12 months by the appropriate licensed medical personnel clearing for athletic particiaption. Responsibility for primary medical insurance coverage rests with the camper. I acknowledge the risk involved in attending any showcase provided by Servant First LLC and I agree to releasing Servant First LLC from all liability to injuries that may occur while participating in a showcase. By signing, I agree to holding Servant First LLC entirely free from any liability including finanical responsibility for injuries incurred.

EARLY BIRD REGISTRATION is $100 for each baseball showcase if received at least TWO WEEKS prior to the showcase. The cost of each showcase POST DEADLINE is $150. If a player decides to register on the day of the showcase, the cost is $150.
