This form is to request the allocation of new codes for ICD10h. Please make sure you have read the guidance in the manual before applying for a new code, and only apply if you are sure that no appropriate code already exists AND if a reasonable number of deaths in your database have been given that cause. If you wish to apply for more than one new code, please submit a new form for each. Please note that new version of ICD10h (including new codes allocated) will not be published more frequently than once a year, but the new codes committee will meet more frequently and you can start using new codes as soon as you have been allocated one by the committee.
