MHS Foster Application

Thank you for completing this application. The information you provide will help us to determine the best match for you and your family. In order to be considered as a Foster, you must: 1) be 21 years or older, 2) have a valid ID with current address and 3) you must be approved through the foster counseling process.

1. I will abide by the mission, rules, regulations, policies, and programs of Michiana Humane Society while I am a foster.
2. If I stop being a foster for Michiana Humane Society for any reason or upon Michiana Humane Society’s request at any time, I will promptly return all of Michiana Humane Society’s supplies, equipment, records, and other items in good, clean condition.
3. I assume the risk of being bitten, scratched, injured, or frightened by: cats, kittens, dogs, puppies, rabbits, guinea pigs, and any other species under Michiana Humane Society’s care in connection with my volunteer work for Michiana Humane Society. Michiana Humane Society is not liable to me for any injuries, damages, liabilities, losses, judgments, costs, or expenses whatsoever which I might suffer or sustain in connection with the performance of my volunteer activities for Michiana Humane Society unless they are the result of Michiana Humane Society’s gross negligence or intentional misconduct. I indemnify, defend, and hold Michiana Humane Society harmless from and against any claims, lawsuits, injuries, damages, losses, costs, or expenses whatsoever sustained by any companion animal or volunteer activities for Michiana Humane Society, or my breach of Michiana Humane Society’s rules, regulations, policies, and programs.
4. I understand and agree that Michiana Humane Society may refuse volunteer applications for any reason.
5. I give Michiana Humane Society permission to contact the veterinarian and landlord I have listed for the purpose of verifying information.
6. I consent to Michiana Humane Society visiting my home or business from time to time to observe the animals and their living quarters and level of care.
7. Any modification to the agreement must be in writing, signed by both parties.
8. This agreement is binding between The Michiana Humane Society, me, and Michiana Humane Society’s and my respective heirs, successors, assigns, executors, and personal representatives.
9. I understand that dogs must remain on leash when outside, unless in a completely fenced area. Michiana Humane Society foster dogs are not to meet dogs outside those listed on the foster application unless previously approved by Michiana Humane Society—including but not limited to the use of dog parks.
10. I understand that any equipment or supplies that I purchase for a foster animal is an expense of my own. Michiana Humane Society will only reimburse expenses that are approved in writing prior to purchase.
11. Michiana Humane Society’s foster cats and kittens must remain indoors unless prior written approval for leash or stroller rides is obtained from Michiana Humane Society.
12. I have accurately and truthfully completed my Volunteer Application and Agreement.
13. I understand and agree that Michiana Humane Society has the right to remove me from the foster program at any time at their discretion.
